Behavioral Health

Most often the familiarity of the home setting provides the best environment to care for patients with emotional and behavioral health disorders.  Connecticut VNA Partners provides goal-oriented, patient-centered solutions - helping patients regain independence in the comfort of their own homes.

Behavioral health services are designed to meet the specialized needs of individuals who may be experiencing a psychiatric or medical crisis involving an adjustment to:

  • The loss of a loved one (depression or grief)
  • Illness/multiple medical problems
  • Cognitive disorders (including Alzheimer's disease and dementia)
  • Suicidal thinking
  • Sleep disorders
  • Substance abuse
  • Noncompliance with medications or treatment plans
  • Prior psychiatric history

The behavioral health program complements community-based care and coordinates services for patients by providing links between inpatient hospital programs, outpatient services, residential housing, group/boarding homes, or respite care.  

The interdisciplinary team uses a holistic approach.  This team includes the Behavioral Health RN, Medical Social Worker, Rehabilitation Services, Home Health Aides, and Homemakers.
